What to Expect
Initial Reach Out & Intake
An intake coordinator from Methow at Home will gather basic details about your situation and care needs to complete a referral form.
Triage & Team Match
Our triage lead and clinical team review the referral form to pair you with the right specialist or volunteer team (such as nursing, social work, occupational therapy, mental health, or end-of-life care).
Contact & Scheduling
Your assigned Lookout Coalition provider will contact you or your family within 24 to 48 hours to schedule a convenient time for an initial visit.
Donate to The Lookout Coalition
Give Methow is an online fundraising campaign that supports Methow Valley nonprofits.
Donate October 1-31, visit www.GiveMethow.org to donate.
Minimum donation is just $10 and they take care of the credit card fees so 100% of your donation directly support the nonprofit.
Donate on a “Funday Monday” and be entered to win additional funds for the participating nonprofit(s) of your choice!
